improve their health, life, and wellbeing. Chronic Care Management (CCM) is a

Medicare program introduced in 2015 to improve patient's overall health. Partnering with Physician Offices, we provide CCM case management services telephonically to help coordinate care between different health care providers, provide education & counseling to patients, and work with these patients to create individualized care plans remotely.
