Our approach
LD Avenue Consulting is equipped to manage a variety of needs to improve your health, life, and wellbeing. Chronic Care Management (CCM) is a Medicare program introduced in 2015 to improve patient's overall health. We provide CCM case management services to help coordinate care between different health care providers, provide education & counseling to patients, and work with patients to create individualized care plans. LD Avenue Consulting also offer consultation and advisement to help individuals navigate professionally.

Owner and Operator, TaShara Anderson, is a Registered Nurse with a Bachelors in Science of Nursing degree from the University of South Carolina. With 15+ years of healthcare experience, she has extensive knowledge of the healthcare system and is an advocate for the community she serves.
